Worker Fatally Electrocuted Resulting in Multiple Violations

OSHA launched an investigation on Tennessee Valley Authority (TVA) Run Fossil Plant following an incident resulting in a contracted worker’s death. The platforms provided for the employees to work on hadn’t been inspected or deemed safe. This led to the employee falling 5 feet through an electrical enclosure, where he made contact with energized electrical equipment, resulting in electrocution.

TVA was ultimately cited with one serious notice, while the two contractors at the plant were cited with fines of $152,900 and $14,502.

According to OSHA, TVA and the two companies were all at fault. They exposed workers “to fall and electrocution hazards by not determining if the walking and working surfaces (that) employees were working from had the strength and structural integrity to support them safely.”

OSHA also decided William Specialty Services “failed to provide safe access to elevated platforms; and failed to initiate and maintain programs that provide frequent and regular inspections by a competent person of the job site and equipment.”
